JOB TITLE: Supported Services Aide

CLASSIFICATION: Non-Exempt- 36 hours weekly

EFFECTIVE DATE: July 2008

SUPERVISION: Supported Living Services Coordinator



--SUMMARY:

The Personal Assistant is hired to assist in planning for, and directly supervising home and community activities for individuals served in Supported Living Services. This includes but is not limited to assisting with: participation in recreational community activities, supervision in the absence of an individual’s primary caregiver, assistance with scheduling and attending doctors appointments, assistance with scheduling and attending therapy appointments, safety training activities, transportation/bus training activities, money management, reading and explaining mail, bill paying, grocery shopping, meal planning and preparation, and the implementation and documentation of skills development.

--QUALIFICATIONS:

* One year experience working with persons with developmental disabilities preferred. A degree in human services or related field may be substituted for experience.
* Must have strong organizational skills.
* Must have the ability to work independently

--ORGANIZATIONAL RELATIONSHIPS:

* Direct supervision provided by the Supported Living Services Coordinator.
* Works cooperatively with Supported Living Consultants, individuals receiving services and their families.

--ESSENTIAL FUNCTIONS:

* Communicate in written and oral form
* Ability to lift 50 pounds using proper lifting techniques.
* Ability to push and pull a 175 pound wheelchair.
* Use a wheelchair lift in agency vans.
* Drive personal vehicle for activities and appointments.
* Maintenance of a flexible schedule to meet individual needs.

--RESPONSIBILITIES:

* Maintain documentation of billable hours and activities.
* Develop natural supports in the community and facilitate positive relationships within the community at large.
* Implement support/service plans as requested by supervisor.
* Complete area specific paperwork (daily, weekly & monthly) as assigned.
* Assist in development of weekly schedule.

JOB TITLE: EMPLOYMENT CONSULTANT      

 

SUMMARY:

Provide and coordinate supported employment services to persons who are served by NMCS in the Employment services department. Services include assessing consumer’s abilities and interest, generating/developing job placements, assist in transition, on site training and on going support. Must be willing to work along with consumer as a role model, and to demonstrate actual job tasks during the training process. The Employment consultant must be able to maintain appropriate employment sites.

 

QUALIFICATIONS:

Valid drivers license with good driving record as approved by assigned insurance carrier. Proof of personal vehicle insurance, high school diploma and one year experience working with persons with developmental disabilities. A degree in human services or related experience may be substituted for experience.

 

ESSENTIAL FUNCTIONS/RESPONSIBILITIES:

Communicate in written and oral form. Access, input and retrieve information from a computer and printer. Adhere to flexible work schedules as determined by individual consumer needs. Use personal vehicle in the performance of job duties. Transport clients to and from home and or assigned program function using agency or personal vehicle as requested. Attend and participate in all meetings regarding assigned consumers as well as any special assignment by supervisor. Assess vocational skills, strengths and weakness, areas of interest, for assigned consumers, write reports as required. Consumer job development bases on assessment to provide and appropriate job match. On site job training. Develop employer/employee relationships with the workplace. Coordinate special services related to employment and special needs consumers. Write and implement individual support and service plans as determined by interdisciplinary team.

 36 Hours per week- flexible hours including some nights. Fridays are mandatory.
