| What does it mean to be eligible for North Metro's services under the age of 18? | ||||||||||||||||
Children from birth to age 3 yearsThese children and their families are offered a wide variety of services through our Early Beginnings program, and, fortunately, State of Colorado funding has been immediately available to meet many of their needs. Children from age 3 to age 18 yearsChildren in this age category are eligible for participation in the Family Support Services Program (FSSP) which provides resource information and funding for therapies, medical and dental co-pays associated with the child's disability, and necessary equipment not paid for by Medicaid or private insurance. Emergency Respite Care funding may also be requested by parents. On a yearly basis, parents of children in this age group are asked to complete either a Family Support Plan (currently accessing FSSP) or an Annual Update document, returning it to North Metro. Re-determination of Eligibility at age 6State of Colorado law requires that the eligibility for continuing services for children at age 6 must be re-determined. School documents, such as a child's Individual Education Plan (IEP) and test results for IQ (Intelligence Quotient) test results are reviewed by our Eligibility Committee. A decision is then made about continuing eligibility. At age 14, a child's eligibility is also reviewed. |

| What does it mean to be eligible for services age 18 and over? | ||||||||||||||||
| This wide age group of consumers may be active in the Supported Living Services Program or the Comprehensive program or may be on our adult waiting lists. Active consumers work with a Resource Coordinator and receive case management services which include at minimum a yearly meeting to develop their Individualized Plan (known as the IP). Those on waiting lists are sent an Annual Update to complete and return to North Metro this document helps our agency keep track of changes in a consumer's situation. | ||||||||||||||||
