This department provides case management services to active consumers of our services or persons waiting for services.

Case management involves linking and coordinating services to ensure that a person's individual needs are met. Our case managers are called Resource Coordinators; they are responsible for the following services:

• Intake
• Needs assessment
• Team development of an individualized plan
• Implementation of that plan
• Case coordination and ongoing assessment
• Record keeping and quality assurance
• Referral or discharge, when necessary

There are three categories of service in our department: Intake, Children's Services, and Adult Services as well as a clerical support team. A bilingual Resource Coordinator is employed in our department and ensures that Spanish-speaking families understand our Intake process, waiting lists, and active services. She is available to these families by phone.
