This State or Medicaid-funded program provides services and supports to persons, 18 years old and over, with developmental disabilities, allowing them to live independently or with family and participate as fully as they desire in their community.

Funds are limited for these services and supports and are intended to supplement, not replace, existing services and supports. Each person enrolled in SLS works with a team to identify, prioritize, and tailor needs and desires that are not available through community/family resources, private insurance or the Medicaid State Plan.
