The Intake Process

The Intake Coordinator, housed in the Resource Coordination Department, is generally the staff person who has initial contact with someone calling about services. This Coordinator asks for basic information about the person being referred and mails a packet of information to the caller. This information includes, at the very least, a North Metro brochure, an application for services, and release for information forms.

Once the application and releases have been returned, the Intake Coordinator sends for documents that will support the fact that the person referred is developmentally disabled or delayed — or will work with the person/family to arrange for IQ (intelligence quotient) testing and, if necessary, other testing.

The next step is to determine the applicant's eligibility as a developmentally disabled person. A committee of 5 professionals well-versed in developmental disabilities makes this determination. The applicant may be found eligible, not eligible, or eligibility undetermined in which case more information is requested. An Appeal Process for those found not eligible is available.

When an applicant is found eligible, he/she is enrolled into the North Metro system and, if requested, is placed on appropriate waiting lists. Waiting lists include Residential Services, Supported Living Services, and Family Support.
